Patient Rights and Responsibilities is a summary of certain rights that you have guaranteed to you as a patient. Some are guaranteed by federal law, while others are state laws protecting patients, and healthcare facilities.  Another document is the Notice of Privacy Practices, this document provides a clear, user-friendly explanation of individuals’ rights with respect to their personal health information and the privacy practices of health plans and health care providers.
